Uncleared PIV
The individual receiving this email has been entered into the NBIS eAPP system. You will receive two emails from NBIS eAPP, one will have your username, and the other will have a temporary password.
Tips for NBIS eApp:
In the education section it is required to enter your high school diploma/GED information. If you did not receive a high school/GED diploma then put an optional comment in the education section.
In addition, your references cannot be listed in the reference section and in the residence, education, or employment section. All addresses that are required to be listed must have a complete address (number, street name, Road, Lane, Avenue, etc.).
For assistance obtaining your selective service number go to https://www.sss.gov. If you were born after 12/31/1959 please go to the link and put your selective service number in the section.
Before you submit your NBIS eApp packet please come to the badge office as soon as possible to complete the enrollment (fingerprints, proof of citizenship, and new photo) or this will hold up your process.
For your uncleared PIV Badge Request, provide the following information to the Badge Office during your enrollment process:
You must bring two non-expired forms of identification from the list below to your HSPD-12 Enrollment. At least one form of identification must be a government-issued photo ID.
- Driver’s License – must be current/ Real ID not required (temporary paper copies cannot be accepted)
- U.S. Passport
- Birth Certificate with seal (or a certified copy) or naturalization certificate
- linking document required if name does not match name on ID
NOTE:
- If you are a remote (not in the Oak Ridge, TN area) employee or contractor please notify us immediately for revised instructions.
- HSPD-12/PIV requests will be cancelled if not submitted in the allotted amount of time.
You will receive an email within the next 24 hours from HSPD12Admin@usaccess.gsa.gov. The automated email will include instructions for scheduling an Enrollment Appointment; however, we request that you “DO NOT SCHEDULE AN ENROLLMENT APPOINTMENT in the scheduler”. Please simply come to the ORNL Visitor Center M – F 8:00 AM to 3:30 PM for your enrollment process.
Once your NBIS eApp has been submitted to DOE you won't hear anything until the badge has arrived at ORNL for you to pickup. You will receive an email letting you know to come and pick up your badge.
